The State of Tennessee requires certain immunizations for all students (undergraduate, graduate, and professional) on university campuses. As such, Vanderbilt University will block student registration for those who are not in compliance with the requirements. In order to accommodate students who have difficulty acquiring their records or needed vaccinations, incoming students not in compliance with the state laws will be enrolled for their first semester, but if they fail to comply within two months of enrollment, registration for the second semester will not be permitted. The state-mandated requirements include: Meningococcal meningitis vaccine (one injection) for all incoming students living in on-campus housing. The law does allow a student to sign a waiver stating that he/she does not wish to receive this vaccination. Click here for waiver form. 2. Hepatitis B vaccine series (three injections) for all incoming students, regardless of housing status. The law does allow a student to sign a waiver stating that he/she does not wish to receive this vaccination (see below). Click here for waiver Form. Measles, mumps, and rubella (2 injections) for all incoming students. Any waivers for this vaccine are very strict, and include only certain religious or medical exemptions that must be approved by the Medical Director of the Student Health Center. For more information regarding this waiver, please call the Director’s assistant at 615-322-2254 or email her at studenthealth@vanderbilt.edu All incoming students should complete a Student Health Center Health Questionnaire that includes further information regarding the state-mandated vaccinations, as well as information on other strongly recommended vaccinations. This Health Questionnaire should be returned to the Student Health Center with vaccination or waiver information. This form also contains important health history information that allows the staff at the Student Health Center to better serve your needs. The option of filling out on-line waivers and vaccine information for the state mandated vaccines only can also be accomplished by using the following website and following the instructions. 4. Use the dropdowns to enter your vaccination information, then click Save.

The Student Health Center will then be able to access this information to document compliance with the Tennessee state laws. However, we still strongly encourage all students to also send the Student Health Center their complete vaccination history so that our records are up to date and so that we have your health history.
